HOW TO USE MEET ENTRY FORMS

Instructions: How to use the Meet Entry Forms.

You will require MS Word software to open the meet entry form documents.  This software comes with all MS Office packages installed on most of today’s systems.  IF you do not have this software, you will need to use another computer that does  so you can enter the meet.

1 . Click and open the appropriate birth year document.  Repeat this step starting at 1. for every entry in multiple birth year categories and the Open Medley Relay Form.

2. DO NOT save the document to your computer (yet) … First … type in ALL your school and entry information and credit card payment etc.  The meet events box has a drop-down menu to select your entry.  Each form allows for several athletes to be entered in each birth category and entry in the 4x100m Relay to be selected.  If you have more entries than 1 page allows in a birth year, go back after all the steps are complete and open another document.  Start again and label this form as the “b” or #2 entry form for that birth year when you save it.

3. Now SAVE your MS Word File to a location on your computer where you can find it.  Label the File with a name that you can identify with such as School Name, Gender & Yr of Athlete Form.
(ie: Alta Vista Girls 2000)

4. Now go to your email service and send us an email to meetentry@ottawalions.com.  On this email attach the MS Word Entry Form(s) and send them to us as an attachment.

5. Look for a confirmation of “entry received” within 24 hours or less.

6. Look for entry information and updates on our web site as the championship approaches and arrive on June 11th with plenty of time to get organized.  Report to the registration tent.
